ABSTRACT

Objective:To investigate the association between serum thyroid level and prognosis of critically ill children with euthyroid sick syndrome(ESS).Methods:The clinical data and serum thyroid hormone levels of 176 children with ESS who were admitted to the Department of Pediatric Intensive Care Medicine at West China Second Hospital of Sichuan University from January 2015 to April 2021 were retrospectively collected.According to the prognosis, the children were divided into improved group and invalid group, as well as basic disease group and non basic disease group, and the differences of thyroid hormone between two groups were compared.The pediatric risk of mortality Ⅲ(PRISMⅢ) scores within 24 hours of admission were assessed, and the correlation between thyroid hormone level and PRISMⅢ score was analyzed.Results:Among 176 critically ill children with ESS, the most common diseases were sepsis(31.8%), severe pneumonia (23.8%) and heart failure(10.7%), respectively.The levels of free T3(FT3), T3, free T4(FT4) and T4 in invalid group were significantly lower than those in improved group ( P<0.05), but there was no statistical difference in thyroid-stimulating hormone(TSH) level between two groups( P>0.05). The levels of FT3, T3, FT4 and T4 were negatively correlated with PRISMⅢ score( r=-0.419, -0.459, -0.341, -0.383, respectively, P<0.05), and there was no correlation between TSH level and PRISMⅢ score ( P>0.05). The common underlying diseases of severe children with ESS were malnutrition(31/98), heart disease(30/98), hematologic neoplasms(15/98), and bronchopulmonary dysplasia(10/98). The median age of children in basic disease group was younger than that in non-basic disease group(0.7 years old vs. 2.0 years old, P<0.05); The proportion of children with underlying diseases in invalid group was 24.5%, which was significantly higher than that of children without underlying diseases (6.4%), and the difference was statistically significant ( P<0.05); There were no significant differences in the levels of FT3, T3, FT4, T4 and TSH between two groups ( P>0.05). Conclusion:In critically ill children, a variety of diseases can lead to ESS, and sepsis is the most common disease.Young children with underlying diseases should be more alert to ESS.The more severe the disease, the greater the decline of FT3, T3, FT4 and T4 levels.When low T3, T4 and TSH occur simultaneously, the prognosis of the children may be worse.Thyroid hormone level could be used as an indicator to evaluate the prognosis of critically ill children, which is needed further studies to explore.

ABSTRACT

Purpose@#Alisol A is a bioactive triterpenoid isolated from the Rhizoma Alismatis. Previous studies have shown that alisol A has anticancer potential. In this study, we explored the effect of alisol A on the growth of nasopharyngeal carcinoma (NPC) cells. @*Materials and Methods@#MTT assay, colony formation assay, flow cytometry, transwell assay, wound healing assay, and western blotting were used to assess cell viability, proliferation, cell cycle, migration, invasion, and protein expression, respectively, in vitro.AutoDock Vina and Discovery Studio software were used for molecular docking. @*Results@#Alisol A inhibited the viability, proliferation, migration, and invasion of NPC cells. The molecular docking simulation assay confirmed that alisol A bound to YAP protein. In addition, alisol A promoted the phosphorylation of YAP and suppressed the expression of YAP in NPC cells. @*Conclusion@#Alisol A inhibited the proliferation, migration, and invasion of NPC cells by inhibiting the Hippo signaling pathway. Alisol A may be a candidate drug for NPC.

ABSTRACT

Unreasonable usage of antibiotics has become a prominent problem in clinic, which directly leads to the emergence of multidrug resistant bacteria infection, thus significantly increasing the difficulty of clinical anti-infection treatment.Relevant researches show that the infection range of multidrug resistant bacteria continues increasing, and its resistance gradually deepens, especially in children.Because of the distribution of multidrug resistant bacteria in dynamic changes, and the types and drug resistance of multidrug resistant bacteria that infect children are different from those in adults, leading to clinical treatment more difficult in pediatric.Therefore, the status analysis of multidrug resistant bacteria infections in Chinese children can promote the standardized use of antibiotics, therefore delaying or reducing the invasion of children by multidrug resistant bacteria.

ABSTRACT

The importance of pressing hand in clinical practice of acupuncture was explored in this paper. Through literature review and clinical experience, the role of pressing hand before, during and after acupuncture was summarized and analyzed. Before acupuncture, the pressing hand helped to diagnose diseases, examine pulse and locate acupoints; during acupuncture, it participated in needle insertion and reinforcing-reducing manipulation; after acupuncture, it played a role in needle withdrawal and regulate human body. It is indicated the pressing hand is a key role not only during acupuncture or reinforcing-reducing manipulation, but also participates in the overall process of acupuncture, which has great significance in guiding clinical treatment and enhancing practice level.

ABSTRACT

Blistering cupping is special as eliminating wind and dampness as well as removing phlegm and blood stasis, and it achieves effects through suction. In this paper we reviewed relevant literature combined with clinical experience so as to summarize its operation attention through exploring the origin, mechanism and application. We divide the progress into the blistering period, the phlegm-stasis-eliminating period, and the escharosis period according to the changes of bubble and the things pulled out. Blistering cupping creates ways to eliminate concrete unhealthy influences through smoothing meridians and collaterals, such as phlegm and retained fluid, dampness and blood stasis. Thus chronic diseases are relieved. Also,we propose the rules of "blistering acupoints being related to disease location as well as the nature of acupoints and diseases". The therapy has been used to treat diseases of respiratory system, osteoarticular, skin and subcutaneous tissue, mental and behavioral disorders, and tumor, among which the effects of intractable diseases of respiratory and osteoarticular systems are definite. It deserves to be further explored and promoted.

ABSTRACT

Objective To explore different intervention methods on the risk factors of cardiovascular disease for women with severe pre-eclampsia history.Methods 78 patients with a history of severe preeclampsia in the hospital from November 2006 to November 2008 were chosen as the research subject.They were divided into the observation group and the control group according to the adopted different methods of intervention with 39 patients in each group.The control group was taken with conventional interventions for obesity intervention,and the observation group was treated with standard behavior therapy (SBT).The improving situations of the related risk factors for cardiovascular disease after three months for the two groups were compared.The effect of different intervention methods on the risk factors of cardiovascular disease was evaluated for women with severe pre-eclampsia history.Results After the intervention,the cardiovascular risk factors (systolic blood pressure,body mass,waist circumference,fasting glucose,total cholesterol/high density lipoprotein) of the two groups improved significantly than before the intervention.But the improvement effect of the observation group was better than the control group,the differences between the two groups were significant.Conclusions The intervention effect on risk factors of cardiovascular disease by standard behavior therapy for women with severe pre-eclampsia history is better than conventional way,which is worthy of clinical application.

ABSTRACT

Objective To explore the nursing effect of strengthened psychological intervention on patients with abortion receiving assisted reproductive technology.Methods 180 patients with abortion receiving assisted reproductive technology from January 2009 to December 2011 were chosen as the research object.They were randomly divided into the control group and the observation group with 90 patients in each group.The control group adopted conventional methods of care for nursing,the observation group received strengthened psychological intervention on the basis of the control group.The assessment results of SCL-90 scale and IDA scale before and after five-day nursing were compared between the two groups.Results The scores of SCL-90 scale and IDA scale after five-day nursing in the observation group were lower than those of the control group,there was significant difference.Conclusions Strengthened psychological intervention for abortion patients receiving assisted reproductive technology demonstrated good effect.It can significantly improve adverse psychological state of these patients.

ABSTRACT

Objective To investigate the influence of psychological intervention on the physical and psychomotor development of children born of assisted reproductive technology. Methods 329 cases of ART born children in our hospital were set as the experimental group,the parturients were given pertinent psychological intervention.254 cases of children of spontaneous pregnancy were selected as the control group.The two groups were given follow-up for 24 months.Their physical condition at birth and psychomotor development during follow - up period were compared. Results Compared with the control group,length of infants at birth,body mass of the experimental group were significantly lower,and the incidence rate of premature birth,low birth weight,NICU stay during neonatal stage were higher.But there were no differences in psychomotor development between two groups. Conclusions ART born children of 24-month old show normal psychomotor development,but there are still more unfavorable physical factors,therefore,pertinent nursing intervention should be given based upon psychological intervention in clinic.

ABSTRACT

ObjectiveTo investigate the personnel structure,academic composition,psychological status,nursing behavior and management of the assistant nurses. MethodsThe age,educational structure,mental state and management of 110 assistant nurses were surveyed in our hospital from August 2008 to July 2011,and the results were analyzed. ResultsAmong 110 assistant nurses,24~26 year- old accounted for 60%,tertiary education accounted for 20.9%,working age mostly between 2~3 years.Depression,fear,worry,anxiety,regret,etc.were the most prominent state of mind of nurses. ConclusionsThe old and new with mentoring,early implementation of psychological interventions,maintenance of their self- esteem and strengthening the humanistic quality training can effectively stabilize psychological state of assistant nurses.The implementation of people- centered care management model not only can improve the overall professional quality of the nurses,but also can reach the ultimate goal of nurses,patients,hospitals tripartite benefit.

OBJECTIVE@#By matching patients with presbycusis hearing aids,hearing aid performance assessment questionnaire to fill out to assess the effect of its use and targeted to solve problems encountered in its use and improve the quality of life of older persons.@*METHOD@#Through face to face way to investigate and analyse patients with hearing aids fitting, totally 30 subjects accepted the analysis, preliminary assessment of the use of hearing aids in patient with presbycusis results and solve problems encountered in its use by using SPSS software to analyze the collecting data.@*RESULT@#HHIE questionnaire on statistical analysis, obtained in patients with hearing loss use hearing aids after the problem is a significant improvement statistical analysis of the SADL questionnaire, the conclusion is relatively satisfied with the overall satisfaction.@*CONCLUSION@#Effects Assessment Questionnaire in patients with hearing aids hearing impairment can be epitomized the disabled after use to improve the situation and understand the satisfaction of patients with hearing aids can be an initial effect as the rehabilitation of a reliable subjective assessment of the impact assessment indicators.

ABSTRACT

Objective To evaluate the clinical significance of peripheral atheral atherosclerosis after treatment by folicacid in combination with mecobalarain in uremia patient using vascular ultrasound.Methods 99 cases of uremia patients with carotid atherosclerosis were treated with folie acid tablets and mecobalamin tablets for six months.The carotid intimia-media thiclmess(IMT),total plaque area,numbers and ultrasound types of carotid atherosclerotic before and after treatment were observed.Results After six months,carotid IMT became tinner significantly,and the totsl plaque area decreased(P<0.05).The ultrasound types of carotid atherosclerotic plagues have changed after six montha.and the plaque numbers decreased significantly(P<0.05).Conclusion Threatment by folie acid combination with mecobalamin in uremia patients maybe helpful to the progression of carotid athereaclerotie disease.

ABSTRACT

ObjectiveTo evaluate left ventricle partial and global diastolic function in patients with silentmyocardial ischemia (SMI) by Doppler tissue imaging (DTI) combined with dobutamine stress echocardiography(DSE).Methods30 cases of silent myocardial ischemia(SMI) and 30 cases of normal control underwent dobu-tamine stressechocardiography(DSE). DTI was acquired at baseline and stress, and waveforms in mitral valve clos- ing, and the base and mid segments of left ventricle were used to measure diastolic early velocity(Ve), diastolic latelyvelocity(Va) ,and E deceleration time(Te). ResultsⅤ in control group increased accompanied with the increasingdose of dobutamine, Ve/Va decreased a little but not significantly. Ⅴ in the group with silent myocardial ischemia(SMI) increased at 10μg·kg-1·min-1 ,but decreased at 40μg·kg-1·min-1 compared with 10μg·kg-1·min-1 ,T inboth control and DSE group decreased at 40μg/kg accompanied with 10μg·kg-1·min-1, T in both control and DSEgroup decreased accompanied with the increasing dose of dobutamine, and there were significant differences betweenthe groups(P < 0.05). ConclusionDTI-DSE could quantificadvely detect the partial and global diastolic function ofleft ventricle.

OBJECTIVE:To study the effect of over the counter dispensing model on hospital pharmaceutical work quality improvement and its social benefits. METHODS: The practice of over the counter dispensing model in the outpatient dispensary in our hospital was compared with the traditional model of drug dispensing through a small window. RESULTS: Over the counter dispensing service model can help improve the work efficiency in outpatient dispensary, strengthen pharmacists' consciousness on pharmaceutical care. CONCLUSION: Over the counter dispensing model can be applied in large and medium-sized hospitals.

ABSTRACT

Aim To study the effects of amitriptyline(Ami)on focal cerebral ischemia-reperfusion injury in rats.Methods An animal model of focal cerebral ischemia-reperfusion injury was induced by the middle cerebral artery occlusion(MCAO) by reversibly inserting a nylon thread method.The rats were decapitated after ischemia for 1 hour and reperfusion for 2 hours.The infarct volumes were determined using a 2,3,5-tri-phenyl tetrazolium chloride(TTC) staining and assessed by image analysis system.The neurologic deficit status were evaluated on 0~5 grade scale.The levels of dopamine(DA),norepinephrine(NE),serotonin(5-HT) and its metabolic product~hydroxyindole acetic acid(5-HIAA) in cortex and striatum were measured by fluoro-spectrophotometry.Results Ami treatment exhibited a remarkable reduction in infarct volume and neurologic deficit scores.The monoamines content of cortex and striatum had a significant increase compared with ischemia-reperfusion group.Conclusion Amitriptyline has protective effect on cerebral ischemia-reperfusion injury in rats.The mechanism might be related to reducing the release of NE,DA and 5-HT during cerebral ischemia-reperfusion,attenuating or inhibiting of the neurotoxic effects of monoamine neurotransmitters.

ABSTRACT

Objectives To investigate the diagnostic value and safety of needle cutting biopsy for the primary and metastatic hepatic cancer during the treatment of percutaneous radiofrequency ablation and ethanol injection guided by ultrosound or CT. Methods The clinical data of 48 patients underwent the treatment of percutaneous radiofrequency ablation and ethanol injection as well as biopsy were analyzed. Results Among 48 patients,46 case(95.8%) were diagnosed by pathological examination and 2 cases (4.2%) failed to be diagnosed. There were no serious complications in all the patients. Conclusion Detachable needle cutting biopsy was safe and effective for the diagnosis of the primary and metastatic hepatic cancer during the treatment of percutaneous radiofrequency ablation and ethanol injection guided by ultrasound or CT.

ABSTRACT

Objective: To study the changes of andrographolides in the drying process of the extract of Andrographis paniculata Nees. Methods: HPLC method was applied to analyze andrographolide and 14 deoxy 11, 12 didehydroandrographolide in the process. Results: The content of andrographolide descended rapidly in the whole drying process, while the content of 14 deoxy 11, 12 didehydroandrographolide ascended at first 12 hours, declined in content was slowly to follow. Conclusion: Baking temperature is not the only main factor to stimulate the transformation of andrographolide and 14 deoxy 11, 12 didehydroandrographolide.
